Staying in Place - A Community Partnership
A collaborative project with HomeMade and Mable to provide our aging community the support they actually need
What is this partnership?
Leongatha Community House is proud to announce our new partnerships with HomeMade and Mable. Through these collaborations, we are now a registered service provider for people accessing Support at Home Packages (formerly known as Home Care Packages).
Supporting retirees has always been a core focus of our work at the Community House. We regularly hear from community members about the challenges of finding providers with available capacity or services that truly meet their needs. When this opportunity arose, we were excited to step in and help bridge that gap.
How can Leongatha Community House help?
If you have a Support at Home package and are looking for a service provider or thinking about changing providers come and see us!
Through our partnerships with HomeMade and Mable, you’ll have genuine choice and control over the people who support you.
If you don’t yet have a Support at Home package but feel you could benefit from extra support, we can help you get started with the assessment process. Come in and chat with us, we’re here to support you every step of the way.
How the partnership works:
HomeMade
The registered aged care provider, ensuring all compliance and government requirements are met.
Mable
Connects you with a wide range of independent support workers. Anyone with the right to work in Australia who meets Mable’s requirements can join the platform.
Leongatha Community House
Your local coordinator. We manage your intake, set up your supports, and organise the workers and services you receive. We’re your direct, local point of contact.
This model of care is all about choice and control, and we love that it’s built around locals supporting locals.
We’ve created a helpful form (linked below) to guide you through the key things to consider when preparing for your My Aged Care assessment.
This resource is designed to help you identify the types of support you may need, feel more confident speaking up during your assessment, and ensure you access the right care and funding for your individual situation.
"Life on your terms"
Continue living life your way with the freedom to control your aged care funding and set your care schedule.
